The Medical Record Unit provides services for the below requests:
- Written medical reports
- Completion of Insurance Claim Form, PERKESO, KWSP, Vaccination Records etc.
- Investigation reports such as Laboratory Result or Radiology Result
- Copies of Discharge Summary, Referral Letter, Executive Health Screening Result etc.
What are the requirement to apply for the Medical Report?
- If you are the patient:
– Application for Release of Medical Information Form
– A copy of patient’s ID (front and back) – NRIC or Passport
- If you are requesting on behalf of a patient :
– Application for Release of Medical Information Form
– Consent for Release of Medical Information (this is to comply with the — Personal Data Protection Act 2010) – to be signed by patient
– A copy of patient’s ID (front and back) – NRIC or Passport
– A copy of requestor’s ID (front and back) – NRIC or Passport
- For Deceased Patient :
– Application for Release of Medical Information Form
– Consent for Release of Medical Information
– Copy of Deceased’s Death Certificate
– A copy of requestor’s ID (front and back) – NRIC or Passport
– Grant of Probate / Letter of Administration
- Where applicable:
– Official Application Letter – i.e. Insurance Query Letter / Claim Form, Lawyer Letter, SOCSO form, etc.
– Written authorization letter from patient (if collected by other than patient or requestor)
– Marriage Certificate (for married couple)

How long does it takes to process the application?
The process will take about 30 working days from the time all required documents are obtained.
However, there are cases where the requests may take longer than the given period. The delay may be due to the unavailability of doctors (e.g. visiting doctors, doctors on leave), reports requested from multiple doctors etc.
How to collect the report?
When the report is ready, you will be contacted via email. The report will be released based on your preference as below:
– Onsite collect at Medical Report Unit @ Level 1 Prince Court Medical Centre
– Scanned copy via email
– Original hardcopy via Courier Services
Note :
For immediate release, it is advisable for you to come and collect onsite.
Online payment will take up to 3 days for verification by our Finance Department.
How much is the report fee?
1. Minimum fee of RM120 will be applied to all applications.
Charges are inclusive of administrative fee and doctor’s medical report fee
2. No charges for copies of investigative report
What is the payment method?
- Cash payment upon collection of report.
OR - Online banking via JOMPAY (further details will be emailed to you when the report is ready for collection)
